If your student has a special diet that requires a substitution, such as lactose intolerant or gluten free, a SPECIAL DIET FORM needs to be filled out and sent back as soon as possible.  This way the health aide or nurse can send it to the head of food services and the new accommodation can be put in place.  This form does need to be signed by a physician.
