If your child has asthma, of any kind, it is important that we have an ASTHMA ACTION PLAN on file, EVEN IF they are not have an inhaler.  This form does need to be signed by a physician.

If your student falls in this category, please click the link below to fill out an Asthma Care Plan.  If your student will be keeping an inhaler at school, please also fill out a Medication Authorization Form.  This also needs to be signed by a physician.
