If your student has any allergy that would be in need of Benadryl, Epi-Pen, or Auvi-Q, then a FARE CARE PLAN must be put in action.  This care plan allows us to know what to do if your student comes in contact with their allergen. 

If your student falls in this category, please click the link below to fill out a FARE Care Plan.  A Medication Authorization Form is also needed if medication will be kept at school.  This needs a physician's signature. If you student will be having School Lunch, please fill out a special diet form as well (below) as well.
