About me
My name is Kelsey Rockstroh and I have been a nurse for 8 years. I started out in Pediatrics at Beacon Children's and then switched over to School Nurse after three years for a more stable schedule. I have a wonderful husband and sweet daughter that is now two years old! I also have a little zoo that consists of a dog, a turtle, and a bearded dragon. I also am a girl who runs on Diet Coke and Hobby Lobby!

Food Allergies Care Plan
Any student with severe food allergies will need a FARE plan. These include any food allergies that require Benadryl or Epi Pen.
2.
Special Diet Form
Any student with ANY food allergies that require special accommodations for the cafeteria requires this form. This would include Gluten Free Students.
3.
Asthma
Care Plan
Any student that has an Asthma diagnosis will need to have an Asthma Care Plan filled out. This is necessary even if the student does not have an inhaler.
4.
Seizure
Care Plan
Any student with Seizures will need a Seizure Care Plan filled out. This is necessary even if the student does not have/require Diastat.
