Immunizations
In accordance with PHM Policy 5320 and state law, you are required to furnish—no later than the first day of school after your child’s enrollment— proof of your child’s immunization status, either as a written document from the health care provider who administered the immunizations or documentation provided from the state immunization data registry. If you do not provide the required documentation by your child’s first day of school, your child may be admitted to school provided that the necessary documentation is received within 20 school days.

If your child remains unimmunized and/or PHM has not received the required documentation by this date, your child will no longer be permitted to attend school and PHM will commence expulsion proceedings and notify the Department of Health and the Department of Children’s Services (unless you have provided a religious objection or submitted a physician’s statement indicating that the needed immunizations are contraindicated).
