Seizures

If your student has seizures, including absent, grand mal, cluster, etc., a SEIZURE ACTION PLAN needs to be on file.  This does need to be signed by a physician.

If your student falls in this category, please click the link below to fill out a Seizure Care Plan.  A Medication Authorization Form is also needed if medication, such as Diastat will be kept at school.
